FAX Cover Page/Cover Sheet: Suggested text for CONFIDENTIALITY

Hello,

Could you guys suggest to me text to be used for "Confidentiality Statement" for a FAX Cover Page/Cover Sheet?

CONFIDENTIAL NOTICE

This e-mail/fax transmission contains confidential or legally privileged information, which is intended only for the use of the individual or entity named in this transmittal. If you are not the intended recipient, you are hereby notified that any disclosure, copying, distribution or reliance upon the contents of this e-mail is strictly prohibited. If you received this transmission in error, please notify us immediately, and please forward the e-mail or fax, back to us with notification that you have deleted the transmission from your system.

That is a good one.

I am not sure if there is any legal mumbo jumbo specific for each state.

I personally don't like these statements ... you're putting the onus on the receiver to correct your mistake.

I would add ", and we shall courier a chocolate fish to you for your time." or something similar.

Oh by the way:

The sender should manually type the patient name in to the subject line before sending the fax out, so if transmission fails, you'll have the patients name in the fax status description area.
