Has anyone Used Kareo - ASP Based PM & Billing Software?

Yes, they are affordable but you can never get anyone on the phone.  You have to leave a message and it takes days to have someone get back to you!

As far as I know they do not interface with any EMR's. 

I looked at them, but only after I was 95% committed to another PM and burned out looking. In retrospect, I wish I would have tried them.

They look like a product worth pursuing. I was particularly concerned with management statistics and I believe they can and do port data to Excel. I believe they are very eclectic in this regard.  (I was looking for data as custom reports on the screen (not excel)  that would allow me to hyperlink back to appropriate portion of program, ie demographics or billing etc).

If you are concerned about support, ask them some support questions before you buy. Also, negotiate a time period where you can bail if  they don't meet your expectations. Get something in writing with respect to call backs for support.

todidoki is quite correct. The last time I looked at Kareo it was impossible to get anyone on the phone. If you are a billing center using their ASP software and you have a problem, good luck. Also, Kareo charges outrageous prices for data conversions. Let's say you currently have a system in place and you have outgrown it (something that happens all the time). You have thousands of patient demographics, insurances, and transactions. While you could finish out the existing claims in the old software, it would be totally awesome to have at least patient demographics moved over to the new software without having to type it all in. This cannot be done with Kareo unless you are willing to take out a second mortgage on your house. Kareo does provide an API to program against, but by the time you hire a developer to do all that you aren't much better off price wise. In my opinion, any ASP that charges anything more than a nominal fee to convert patient demographics from a new client's existing system to their own sucks ass. Getting at least demos into your ASP software is one of the quickest ways to ensure up-front customer satisfaction with the new customer and get them using your ASP right away so they - and you - can start seeing benefit right away. Instead, what I'm seeing is data conversions are usually the first way ASP's stab you in the back. JamesNT

Here are two quotes from a respected HIT vendor NOT directly involved with providing EMR software.  Might help to understand the dynamics re: data conversion.

"This is not an easy task considering the many and varied EMR systems available. The difficulty is further compounded by the idiosyncrasies of each EMR vendor´s implementation of HL7. "

The word, "idiosyncracies" was a polite way of saying, a mess.  Now with new government standards looming, the landscape is further complicated.
I never see a provider question the software code -- is it truly OO (object oriented) and extensible. Are the data models properly normalized and somewhat standardized to HL7 2 on the way to 3 and ASTM now.  Very simple stuff to evaluate -- CS101. 

Interoperability -- how will these systems talk to CMS and each other down the road?

And finally, guarantees or not, can the code and data be re-factored to meet 2011 ARRA criteria and standards, and the landslide of changes likely to come thereafter -- without downtime for the users and BK'ing the vendor?

Providers often want screen shots or demos, but they NEVER ask for snippets of the code or database views.  Be sure to look under the hood.

Jack, I do understand the idiosyncrasies of doing a database conversion. I've done several over the past 3 years. I have also interfaced with Amazing Charts, PODMEDS, and Medisoft using SQL Server Integration Services. Believe me, I understand that most people's data is pure crap. Missing birthdates, misspelled last names, incorrect genders, wrong modifiers, etc, etc, etc. I have also written an ANSI 837 generator and an ANSI 835 parser that parses the file and posts payments (including line-item posting and deposits) using SQL Server Integration Services. Again, I understand the issues of how people screw up these standards (and I've made my fair share of mistakes). Standards are wonderful right up until someone misinterprets the standard, the standard doesn't allow for something you want to do, or is ambiguous in some way (which is why I'm still laughing at Murdoch over IE, Firefox, and web standards). Most EMR and Practice Management system databases are NOT properly normalized. Of this, I am well aware. Therefore, I have no problem with an ASP charging a fee to convert data over. What I do have a problem with is Kareo, and other ASP's, wanting the data in their own file format they already have a parser for and still wanting to charge several thousand dollars. With Kareo's, and most other ASP vendor's approach, the client does all the work converting to get the data into the file format the ASP is asking for only to have the ASP still want to charge outrageously for importing the data. Kareo wants $1500 just for demographics (that's what the sales dude told my client) after I converted the data out of the client's database into Kareo's file format. Now, if Kareo said to give them a copy of the whole database and they would handle the rest, I'd be all for it. The price would be more than reasonable. But as it is, they want you to do all the work and still charge $1500. I could see a few hundred bucks, but not $1500 - again, just for demographics (i.e. names, addresses, birthdates - not charts) for a one time conversion - not ongoing interoperability. To Summarize: 1. They get you to do all the work putting your data into their file format. 2. They still charge out the ass for what is a simple import to them. By the way, my major is computer science focusing in operating system design and relational theory (a.k.a databases). Please excuse me, I have to go finish laughing at some of the software design decisions the doctors on this forum are making. JamesNT

We are a large billing service (close to 200 providers), and we also run our own in-house psychotherapy program, with 14 clinicians in our group.  We have been using Kareo for some time, and we are very pleased with it.  We have found it to be highly intuitive, and the support has been excellent.  We have found them quite flexible, and open to our suggestions.  We espcially appreciate the fully integrated claims submission process, as well as the detailed level of access features.
There is no charge for support cases related to electronic claims submission and setup issues, and while the support is mostly e-mail based, we have found that when there is an issue that is difficult to resolve by e-mail, we have received a phone call from them.
They also provide the ability to download the data directly into an Excel spreadsheet, which allows almost unlimited reporting features, as well as an easy way to backup your data.
I highly recommend this program both for billing services and medical practices doing their billing in-house.
