I have been lurking in this forum for about a month and have found the comments posted by the members in this forum very helpful.
I am in the process of setting a Medical Billing office and also in the process of going through various softwares. The journey has been a month and the more I check the more it gets confusing. I have started with the screening process and doing demos. My clients are doctors with Internal Medicine Practice and an Urgent Care. Most ASP look very good in demos but how is the actual performance when it comes to real time data processing. My other concern with ASP is the security of the data.
The websites for softwares I have previewed are:
1. Lytec and Medisoft - Server Based
2. Emedpro - Server Based
3. Edgemed - Server Based
4. AdvancedMD - ASP
5. Claim Gear - ASP
6. AllegianceMD - ASP (I understand they do also offer a server based option and I have to check with them on that)
I would appreciate if current satisfied/dissatisfied users of these systems come forward and share their comments, to help me in this elimination and zeroing process.

this post is only a test ... to try to see how EMRupdate changes the formatting of my posts so as to avoid reformatting.
further EMRupdate testing for format retention, this time by converting the Word text to html format, copying that and pasting into EMRupdate.

I believe the main problem with computerized physician order entry (CPOE) is how long it takes to get your work done. E.g, select medicine/ select strength/ select route (oral, ocular, skin)/ select frequency/ select quantity/ select refills/ add additional instructions. Then write it again in your medical progress note.
Learn a faster better way for yourself: using MS Word,
"amo242" can give "Amoxil 250 tid #42"
or
"cia1" can give "Cialis 10 mg prn relations, do not repeat within 48 hrs #5, mr12; caution may cause low blood pressure." There is a standardized shortcut method of nomenclature. We can talk about it sometime here. It's also visible at the website and in the program noted below.
And the prescription can be seized right from your progress note using MS Word. No need to write it twice.
And for refills, click one of patient's meds, a few more clicks/ click the next medicine and a few more clicks/ etc. until finally completed. Faster is click a button that seizes patient's entire list of meds and another button completes the entire task.
Everyone in internal medicine or family practice knows there's no spare time to fiddle with the computer. It needs to be like greased lightening.

Dipty, sorry that Gil has taken over your post. I used Medisoft for years - thought it was decent, inexpensive software. Started using GE Centricity Practice Management 2004 (formerly known as "Millbrook") over a year ago and it's hands down the best billing program out there. I'm fairly confident whatever you wanted to do collections-wise, this app could handle it. I'm constantly finding new features. Our hospital is also using it to manage several primary-care offices and I asked the girl managing it how she liked GE over what she had been using (Medisense & NTerprise) and her reply was "100% better."
Apologies. Obviously, I could not find a place to trial posting EMRupdate. Hopefully it at least moved the subject to highest on the list of postings about billings, ... but regardless, ... Sorry. There's probably a place at EMRupdate to send questions. I shall see if I can find one.

Any luck on your search. Which ones have your found to be better price, more effective. We use Lytec 2005 and runs smooth but having problems transmitting the data to our ENS clearinghouse lately and thinking about changing as my biller has some issues with Lytec.
I like Practice Admin. I have been using them for 4 months now and find the program to be good.
It is an asp at $250.00 per provider per month. They send out the statements for .43 cents per page and they include a return
envelope. They were submitting all claims themselves but have now changed to Instamed. Add them to your list to
check out.
Edgemed- I checked them out and liked the program- but, didn't want server based software.
AdvanceMD- looked good, but to expensive
Lytec- An Office Manager friend of mine doesn't care for Lytec- buggy.... And again server based
Medisoft- It seemed as though I had to do all the updating and it was server based.

Looking for a medical billing software with all of the perks takes up a lot of time for anyone. First I would recommend assessing what your billing service/office needs are going to be and then review the following check list to see what is most important to you. Once you have that information I would recommend visiting this website I found http://medicalbillingsoftwarepro.com/Medical-Billing-Software-Reviews.html It has 85 medical billing software vendors (40 are web-based products) including which ones are web-based, server-based, EMR/EHR, UB04 the year in business and State location. When I completed my last research this website actually saved me hundreds of manual hours and I was able to identify the software I wanted:
Analysis of what a billing service wants to offer clients:
1. Appointment Scheduler - what functionality does your doctors need to perform their front office jobs
2. Web Based or Server Based (for billing services most recognize the power of web-based so anyone around the world can access the data, you own and control the data, no more visits to the office to pick up packets of paper have them upload into the software for instant access)
3. Co-Branding - some web-based products offer co-branding so you can put your logo on the log in page and on the software (it looks more like your software and you can market it this way)
4. HL7 and API interface technologies allows for doctors to have unlimited choices (you know they like choices): Charting products, EMR products, Medical Products, EHR products or create your own product to interface with your practice management software
5. Reporting - Ad Hoc options, # of reports, details in the reports (really compare reports from the different software vendors because some can cause you to re-think your choices)
6. Collections Module - Most offer them but really nail down the functionality of the collections module is it by visit, responsible party, patient? Also find out how integrated it is with the practice management software. Is it easy to identify what is being done, worked and does it offer a complete package or is it riddled with issues that you find you won't actually use it (unfortunately you may not find this out until you actually purchase and use the software).
7. Forms, Templates, & Labels - can you create your own claim templates, forms, labels, encounter forms/routing slips and documents with ease?
8. How many billers does it take to work charge amount $100,000. Once you know these #'s you can identify the efficiencies of the software.
9. Can you grow into your software?
10. # of users that can be added to your software?
Web-Based software really is amazing in it offers flexibility and security. They run the back ups, protect your data from fire and theft and they purchase the servers so you don't have to. You can log in from hotels, cruise ships, India, or anywhere else you have internet access without worrying if the VPN is working or accessible. They also created their software to interface and transmit claims electronically so you don't have to worry about the server-based connections and upload (saves the office time from manual steps). Of course you do pay for their services on a month to month basis and usually pay 1x setup fees. Example: You are adding a new doctor to the office you will pay a 1x setup fee of x amount of $$'s and then the monthly fee begins. Web-based companies are constantly enhancing their software so you get the perks of what the bigger clients need or are asking for.
Server Based I would weigh in the fees for servers, IT person in the office and all trips and mail services needed to get the data/information to your billing office. How much is your time worth?
