Hi everyone, are there any good integrated EMR+PM+appointment solutions out there nowadays that are pretty inexpensive (<$4000) for a solo practitioner?
Also, I'm working on an appointment self-scheduling solution of my own... does anyone know whether any of the big EMR systems offer an API/interface to export/import patient profile data?

Many of the larger EMRs will support CCR or HL7 export of patient demographics. As for an API .... no idea.

Hi Graham, thanks for the info, I'll look into CCR and HL7. (And thanks for the donation for the MediSoft export guide way back when).
Nothing's really "wrong" with the combo, it's perfectly serviceable and business is working, I just think something more integrated would be more efficient. I'm under the impression that the fully integrated systems are uniformly very pricey, though...
And maybe it's best to let sleeping dogs lie, but I'm the kind that likes to prod :-P
I think that's my impression as well - unless you go for one of those PMS's that let you use their EMR for free as long as they do your collections.
In addition to Medisoft ( based on the information on your walk thru ) I've also written an export from Ezclaims ... and Synapse certainly comes into the inexpensive solutions class.
As an example of its flexibility, I setup a Synapse server on Amazon's EC2 yesterday, and attached a single EBS volume ( equivalent to raid 1 ), shut it down and then restarted it, and connected to it using a local Synapse client. Saves a few dollars from purchasing a high availability local server if you outsource the hardware
Yeah, but you probably pay an equivalent cost if you go the free + billing charge route, at least eventually.
Ahh cool, Synapse seems pretty cool for the DIY'er. I'm a big fan of AWS, although I haven't had reason to use more than S3 so far... the baseline cost for an EC2 instance running at all times to be on standby as a webserver is a bit too high compared with Slicehost, MediaTemple, etc VPS's for low volume. For large production apps, though, the ability to automatically scale up and down is extremely cool. Deploying an EC2 image and maintaining an instance is a bit outside the realm of skill of someone like the doctor I help out, also... :-)
Slicehost is pretty cheap but it's Linux only. Amazon also has a HIPAA policy. And once you scale up then Amazon seems to be cheaper .. eg. a 8gb slice on slicehost might be more expensive then a similarly configured large instance at 50c/hour.
Deploying a public AMI is not that difficult .. and it's pretty much point and click these days. You can shut down your EC2 instance to save a few $ too. Doesn't need to be running on standby.
I think the comparison though is with having your own server, and backup system, and the need for onsite IT support vs just running virtual servers in the cloud.

What does your initial cost (< $4,000) include? Just the software? Server (s) and associated OS? Tablets/PCs? Interfaces? Other?
What do you consider "pretty inexpensive" for ongoing expenses?
What would you include in the ongoing expenses? Maintenance and support? New releases/upgrades? ePrescribing? Other?
Do you want a CCHIT certified system or don't care?

The $4000 would just be for the software, we have all the computers we need.
I'm mostly looking for online alternatives - web applications, SaaS, whatever you like to call them, so I suppose my $4000 number is a bit irrelevant assuming the webapp solutions don't have any setup fee. Basically I'm wondering if there are any all-inclusive solutions that are ~2000/year or less, that handle billing, the EMR side, and appointment self-scheduling.
Don't care too much about CCHIT, but it would be nice (I assume it'll be necessary for to receive federal money...)
While you may be able to find a solution for less than $2k a year, I think it will likely be non-CHIT and/or without all features or all support. When getting prices I would ask "what else do you offer that is not included in this price?" and "what else will I need to run this product, submit claims, meet the ARRA/Stimulus requirements, etc?".

I am sure you can find a combo product for less but I would make sure nothing you need is left out of the price and don't underestimate the value of available, qualified, engligh-speaking customer support.

Inexpensive EMR? A very familiar sound that I hear so often from prospects... I am not clear on the specs/requirements you are looking for but I will point you in the direction of an inexpensive web based solution. Webdmemr. No flat rate, you only pay for what you use. Now that's a global sweeping, worthless and wrong statement from a competing most likely overpriced vendor! How can one be "CHIT"?
At sermo there is now a CCHIT-certified SPAMMER (2008 criteria)! Kind of lets us know about the quality of the CCHIT 2008 criteria. Every posting that he puts up is an advertisement, pissing off everyone on the site, calling for his removal. But I can't blame him- he's using CCHIT for what it was intended for- to try to increase sales (rather than increase performance or quality).

Have you seen a system that offers a good integrated implementation of the basics (EMR, PM, Billing, Appointment Self-Scheduling) for less than 200/month (solo practitioner)? Something tells me the MS Office EMR project is a bit less than 200/mo, but perhaps a bit barebones :-P
