I had an ex-nurse with mild cognitive impairment that didn't want to keep with her insulin injections so she squirted it in her mouth.

 

My EMR is: Synapse It is what we know already that often prevents us from learning.  Pioneers are the ones with the arrows in their backs.

In residency, one of our pediatric patients did exactly that when asked to demonstrate how he used the inhaler.  As ridiculous as it sounds, his pediatric pulmonary visit that day ended with an excellent teaching experience (all around) and no further admissions for months (at least).

 

I like to show this video to my patients on my iphone.  It's just long enough to allow me to write their inhaler prescriptions in peace while they watch, and about once a week I can see some surprise in their eyes about how to use an inhaler or how to use a spacer.
