We like talking patient safety around the Medical Quack by all means and throw in some potential solutions when idea pop into my head. It is technology that is creating a lot of this with distraction or disruption as well. Clinicians are overloaded too with information and they can’t rely too well on what’s in their heads for all the information any more. I learned that a long time ago as a sales person and was an early adapter of PDAs and Windows Mobile Devices, it made me better at what I did and I had my information with me and available. Mr. Quaid returned to the hospital and followed up as featured on Oprah.

It also talks about bar coding in the hospital which is great, but we need to go a bit further than that today as people are getting implanted with devices that have been recalled…..and they die. This needs Tags and there’s a ton of posts I have made on this blog and I started out doing this last October. I write emails to the FDA and some of the drug and medical device companies when I can and have time. There’s no interest other than just trying to crack open a “dead” head somewhere and get some attention here to invest in some technology that will result in saving lives where medical errors would be at fault.

There seems to be some resistance here in responding, but that’s ok, I’ll keep at it as tech and science denial is big. If you go at it long enough someone will pay attention. Watch the video and pretend there’s a box with a stent in it that has been recalled, would it not be nice to see that appear on the screen instantly???

Scan that knee, hip, defibrillator before you use it, takes a few seconds and will help hospital registries function and less mistakes. Recalls are growing and we need to handle this better, is more information that needs a simple and easy workflow, like the Tags.

We all talk about aviation and compare to medical and yes that is important as a friend of mine years ago had to react quickly as their 747 almost crashed over downtown Hong Kong with a left wing flap failure, it wants to roll when things like that happen, so that is really being on the ball and I have had a little time to play around a bit in a FedEx simulator years ago too, so I got a taste of what he did. Speaking of our heroes though too, it was sad to hear that Scully, who landed the jet in New York had his retirement cut by 40% too, what are we doing for our heroes anyway sometimes, kind of sad. WASHINGTON — As a private pilot, actor Dennis Quaid was struck by the differences between how aviation errors and medical errors are handled.

The airline industry doesn't have much choice, Quaid noted in an interview Monday after speaking at a National Press Club luncheon. "When a crash happens, it's so public," he said. "No one is going to fly on their airplanes unless they have that trust."

But when a mistake occurs in a hospital, the public might never hear about it. Although an estimated 100,000 Americans die each year because of medical errors, their deaths are scattered over thousands of hospitals, "where people die anyway," Quaid said. "It doesn't get the same type of attention."

He has produced a documentary, Chasing Zero: Winning the War on Healthcare Harm, which makes its debut April 22 at the Global Patient Safety Summit in Nice, France, where Quaid will be the keynote speaker. It airs April 24, May 1 and May 8 on the Discovery Channel. Quaid also co-wrote an article in the March issue of the Journal of Patient Safety, "Story Power: The Secret Weapon."

Nebraska medical malpractice attorneys alert- A toddler is dead after receiving a lethal dose of Heparin at Nebraska Medical Center.

Omaha, NE—A Dallas, Texas toddler was tragically killed by an overdose of a blood-thinning drug, Heparin, while at the Nebraska Medical Center. The little girl was recovering from multiple organ transplants and gastroschisis when she was given the lethal dose of medication, as reported KETV.

It was determined that the girl wad given too much Heparin. A Nebraska Medical Center spokesperson confirmed, “the hospital staff somehow gave the girl too much Heparin, and the hospital is doing a extensive investigation.” The hospital is reportedly trying to do everything they can to help the mother and father cope with this tragedy. It is unknown if police officials will be conducting a full investigation as well.

How Kaiser Permanente Does Medical Records a Look Inside – Challenges for Others

CBS did a very good in depth report here and one of the most interesting portions is the actual look at a Kaiser facility in Santa Clara, showing some real time use and answering some questions on how and why it works. Yes they made one huge investment and I remember all the information on the internet about how they took strike 2 and went at the project a second time by working with Epic Systems, once their own internal project failed to meet the needs, so you try and try again. Training is mentioned as key and they have over 3 million patients who can access their medical records online too.

Not too long ago I had the opportunity to interview Dr. James Leo at Long Beach Memorial Hospital on their transition to electronic medical records and they use the same Epic System that is used at Kaiser. It was enlightening and I learned quite bit first had there as well as to the plotting, planning and training processes that have to take place for success.

Also shown here is Dr. Blumenthal, the Executive Branch Czar on electronic medical records, discussing the stimulus money and the upfront investments for physicians to participate. In addition, there are several clips from Dennis Quaid, who, not necessarily by his own choice, became a spokesperson for technology and records as relates to safety and medical errors.

Also, what is interesting is to see how much or maybe little our Congress actually understands about the process to in their own video posted on the web back in January, where a representative from Kaiser and Microsoft try to explain how it works, none has seen a PHR at that point. BD

The debate over health care reform is proving to be a no-holds barred battle but, as New York Times' David Pogue reports, electronic records have already provided one big change.

This is great and nice that celebrities are also speaking out. The related reading below has a history of all the past articles about Dennis McQuaid’s story and problems at the hospital, medical errors. We all have so much on our plates today, there is no way we can manually handle the workloads and not be subject to errors, the computer can do that better than a human in a much shorter length of time.

If you hang around this blog enough, there are many posts to read, not only about technology updates, but clinical and personalized medicine as well. All the elements need to come together and technology is the key to making it all work. BD

CHICAGO – Actor Dennis Quaid brought the importance of electronic prescriptions and bar-coding close to the heart with the story of the near-deaths of his newborn twins from a medical error in 2007.

Quaid, Sunday's opening keynote speaker at HIMSS09 and founder of the Quaid Foundation, said he hopes many lives will be saved as a result of his children's ordeal.

In the past few months, Quaid has testified before Congress and appeared on Oprah Winfrey's television show to promote his cause.

In a large hall filled nearly to capacity, an emotional Quaid thanked the scores of attendees "from the bottom of my heart" for their dedication to advancing health IT.

"You are the leaders and I have faith that you will lead us to a better and brighter future," Quaid said.

Two of the biggest causes today for medical errors are lack of information and distraction, yes distraction can very well do it. As we are all so much busier today with tough economic times and the additional flow of information to handle, we need “good filters”, as earlier in another post I had declared the year 2009 as the “year of the filter”. Any of us can be distracted at any time and make a mistake, but healthcare is where we don’t want this to occur.

Actually in an earlier post, Kaiser has a low tech solution as they found the best solution was to have the nurses simply wear a yellow reflective strap around their chest that warns others to leave them alone when administering medications. That is a solution worth a look, but what happens when we all start wearing yellow ribbons so as not to be disturbed?

Tuesday, March 10th is the day to watch Oprah. The related reading links below have additional details on posts from the past on how the entire scenario evolved. Many forms of technology also come to the rescue to help us today as well, whether it’s a new system at the hospital pharmacy that helps check and monitor medications or an electronic medical records system that sends out alerts when prescribing. Just yesterday I posted on the latest technology update at Cedar Sinai.

One thing we can all do for ourselves and to help the doctors and nurses who take care of us, is to get an online Personal Health Record. When you go to see a doctor, he/she is looking for credible information and this is done easily and for free from Google Health and the Microsoft HealthVault. Let those doctors see what you are allergic too and what your health history is, they can make better decisions, and this is especially needed in the ER room. With the stimulus package, everything is in motion as far as funds to get the hospitals communicating. In an interview done recently, with Steve Shihadeh, VP of Microsoft Health Services, we discussed their Amalga software solution and the success it is seeing in connecting hospitals in Wisconsin, initially all the ER Rooms. The lawsuit against Baxter still to come, but this goes along with really needing technology and trained staff to work with such in hospitals and doctor’s offices. This could happen to anyone, anywhere.

With the deluge of information just due to changes in healthcare and the availability of additional clinical information, drugs, etc., there’s just no way we can keep as many things on top of our heads, and with the big level of distractions that exist today, technology can help. Recently at Kaiser big yellow signs were placed on selected nurses to warn others not to disturb when they are administering medications, so between a little old and new technology, perhaps a safer environment can be created. All is takes is one distraction and anyone could make a mistake.

On the other hand with the labeling, I have addressed this a few times on the blog, and this case not only involves the case of origin for the product, but also similars, the wrong bottle being used by mistake with different strengths. Not too long ago I did a story on a hospital that made changes in their trays that emergency staff uses and with the new products, which fit differently, they realized right away that someone out of habit might still look in the old location, thus big labels were added so help reduce a potential problem even before it happened. Dennis Quaid is also a guest speaker this April at the annual HIMMS convention.

Dennis Quaid and his wife Kimberly reached a settlement with Cedars-Sinai Medical Center for $750,000 over a medication error last year that nearly killed the couple's twin infants, according to papers filed Monday in Los Angeles County Superior Court. Nurses at the hospital mistakenly gave twins Thomas Boone and Zoe Grace 1,000 times the recommended dose of the blood thinner heparin, leaving them vulnerable to uncontrolled bleeding and leaving them, for a time, in critical condition.

The conference is being held later this year, and April certainly can bear a little bit better weather for Chicago than February. “Keep your schedule on track for HIMSS09 in Chicago from April 4-8, 2009. The 2009 Annual HIMSS Conference & Exhibition comes just 5 weeks later than last year's conference, held in Orlando, Fla. at the end of February”. Key note speakers this year include Dennis Quaid, which everyone remembers his incident at Cedars Sinai with the overdose issues of heparin earlier this year and now he has become a safety advocate and is in the line up for this year. The site also has a place where readers can contribute their stories as well. Nice to see celebrity action involved here as medical errors can affect everyone and occur more frequently sometimes than what you may believe. You absolutely need to be the “informed patient” today and learn as much as you can. Technology is flowing in to health care at such a rapid rate with changes and new devices, etc. that it is becoming complicated and difficult for the good people we trust our health to, to keep up. Ignorance is no longer bliss and what you don’t know can kill you. Get a personal health account online set up and start saving those records and documents as you may need them someday, sad to say but today it is a “buyer beware” environment in healthcare so read and do the best you can to be the “informed patient” or the “informed family” BD
